Rising defect escape rate. If the percentage of bugs reaching production is increasing over time, your automated testing and code review processes are degrading. This often happens when teams are under deadline pressure and start cutting corners on test coverage.
Increasing cycle time on bug fixes. If bugs are taking longer to resolve, it usually means the codebase is accumulating technical debt that makes defects harder to isolate and fix. This is a leading indicator of broader delivery predictability problems.

Bug fixes introducing new bugs. This is the clearest signal that test coverage is insufficient and the codebase has become fragile. In regulated industries, this pattern is particularly dangerous because it indicates that changes can’t be made safely.
When these patterns emerge, the response shouldn’t be to hire more QA engineers or add more process. It should be to examine the flow metrics. Where is work in progress too high? Where are handoffs creating delays that force rushed work? Where is the architecture creating coupling that makes isolated changes impossible?
